REGISTERED PROFESSIONAL NURSE- one (1) permanent RN position in the NY Connects Unit at the Department of Aging and Long Term Care. Salary Range: $24.66-$31.31/hour (NYSNA Salary Schedule B-Grade 1 - Registered Nurse)
GENERAL STATEMENT OF DUTIES: This position is responsible for providing professional nursing and social assessments to individuals in the community. This includes formulating and carrying out a plan of care to meet the problems and needs of cases assigned. Work is performed in accordance with professional nursing and social work practices. Does related work as required.
TYPICAL WORK ACTIVITIES: The following work activities are listed as examples only and in no event shall an employee be limited to only those examples listed here:
. Investigates reports of possible Adult Abuse and Neglect.
. Completes initial and recertification assessments for home services such as the Personal Care Program (PCA), Expanded-In Home Service for the Elderly Program (EISEP), and the nutrition program.
. Maintains an ongoing caseload of Adult Protective Services(APS), PCA and EISEP cases. Provides ongoing case management services.
. Provides counseling to motivate and increase individual capacity, self-sufficiency and confidence in their ability to handle problems.
. When necessary, pursues court intervention and prepares court documents on behalf of vulnerable adults.
. Provides Financial Management services to APS caseload as needed.
. Completes Patient Review Instruments (PRI).
. Completes all related paperwork within required time frame.
. Takes calls from the community for information and referral.
REQUIRED KNOWLEDGES, SKILLS AND ABILITIES: The RN must have a good knowledge of nursing and social work principles; possess some knowledge of Federal, State, and local public welfare programs; the ability to follow technical oral and written instructions; the ability to keep records and make reports; ability to establish and maintain successful relationships with people; ability to operate a motor vehicle. The person must also have excellent assessment skills, good powers of observation and analysis, initiative, tact, emotional maturity, and good judgment. Finally, the person must possess the time management and organizational skills necessary to successfully manage a caseload, while completing timely paperwork.
MINIMUM QUALIFICATIONS: Possession of a license issued by the State of New York to practice as a Registered Nurse.
