Two Categories of Awards
Part of the mission of the Steuben Rural Health Network is to promote successes, best practices, and quality services of health and human service care in Steuben County. We achieve this in part from our community-nominated awards for individuals or organizations in two categories:
1. Outstanding Service to Rural Health Award – Individual or Organization Award Criteria
2. Distinguished Award in Human Services – Individual or Organization Award Criteria
Individual Awards
Award recipients are selected based on their compassion, creativity, passion, and cooperative attitude in seeking ways to make lasting contributions to rural health care and human services. The nominee’s work should have made a positive impact in Steuben County and have a demonstrated effect on rural healthcare and human services.
Organization Awards
This award recognizes any program or organization serving Steuben County that has improved access to health and human services and information for rural people through innovative, comprehensive approaches. Factors considered include outreach, preventive health and education, quality and efficiency of care, and strong community support and involvement.
Award Entry Narrative:
This is the primary instrument on which the nomination will be judged. Describe in a brief written statement why you believe the person or program/organization being nominated should be considered for the selected award. Narrative should be based on the criteria listed within each award category. Please include the significance of the work of the nominee to the community, populations or individuals served, and explain how rural people have benefited.
Relevant Attachments:
Attachments may be included that enhance and support the award entry. Send copies as materials will not be returned. Supporting letters are encouraged and are limited to three.
