AIM Independent Living Center - Corning NY

Provide Service Coordination to individuals with traumatic brain injuries and individuals transitioning or diverting from nursing homes.

Qualifications:  Previous experience in TBI or NHTD as a Service Coordinator with a current NYS DOH certification is preferable or an individual with a high school diploma with 5 years experience or a BA degree with 2 years of experience of providing service coordination for individuals with disabilities and/or seniors.
