The Steuben Rural Health Network is proud to announce the dates for the Stanford Chronic Disease Self-Management Program, Living Healthy Steuben! Living Healthy Steuben! is a six-week course that is led by two certified Peer Leaders.
Living Healthy Steuben! is a program that inspires individuals living with a chronic condition to take charge of their health and well-being and to maintain an active lifestyle. Friends and family members of those living with a chronic health condition are also encouraged to attend.
Weekly topics will include but are not limited to: Nutrition, Fitness, Stress Management, Pain Management, Evaluating Medication & Treatments, Communicating with your Health Care Provider and establishing a Health Care Proxy.
Classes will be held at the Hornell Senior Center in Hornell, NY. They will start August 3rd and run till September 7th, meeting every Wednesday. Classes will be from 1:00P.M. till 3:30P.M.
If you are unable to attend the August class, there will another set of classes that will be held at the Corning Senior Center in Corning, NY. That set of classes will begin September 21st and run till October 26th. Those classes will be from 1:00P.M. till 3:30P.M.
A healthy snack will be provided at each class.
