Service Coordinator (Corning, NY)

Full time (35 hours per week) Service Coordinator is needed to work with elderly and/or disabled residents in an independent living facility in Corning. Bachelor's degree in Social Work or related field plus two years human service experience is required. Experience working with elderly adults and/or handicapped adults and knowledge of local resources and case management experience highly desirable. Position requires excellent interpersonal skills, prompt and accurate documentation of services, ability to write required reports, and ability to maintain confidentiality of residents.

Job duties include maintaining regular contracts with residents and their family members in order to make referrals to appropriate community services for residents, creation of a monthly newsletter for residents, completion of quarterly and annual service reports, working closely with the elected Resident Council to improve the satisfaction of residents, planning speakers and small events for the enjoyment of residents, and working cooperatively with employees of the housing facility and the facility's Board of Directors to assure the coordination of services needed by residents.
