The work involves responsibility for reviewing local jurisdiction emergency health programs.  The work is carried out in accordance with standard, well established public health techniques and procedures as recommended by the New York State Department of Health and involves planning, directing, developing, administering and evaluating the public health emergency service in relation to the total program.

Responsibilities include communicable disease & rabies investigation and response, maternal child health home visits, vaccine administration and other public health related duties as assigned.

Assist consumers in meeting their employment goals.

The primary goal of the Family Support Navigator is to assist families and individuals with gaining an increased understanding of the progression of addiction and how to navigate insurance and treatment systems.

Assist individuals and families with the enrollment into AIM’s Consumer Directed Personal Care Services and conduct outreach to promote the program and reach new participants.

To advocate for individuals with developmental and other disabilities to lead a self-directed life.  We believe people with disabilities deserve the opportunity to direct their own supports while maintaining or improving their quality of life in their homes and in their community.

Service coordination assists persons with development disabilities and their families in gaining access to services and supports appropriate to their needs.  OPWDD delivers almost all service coordination through its Medicaid Service Coordination (MSC). MSC is provided by qualified service coordinators and uses a person-centered planning process in developing, implementing and maintaining an Individualized service plan (ISP).

To assist individuals with disabilities to remain home independently in the community by arranging and administering consumer directed care.

This position works full time in the Elmira City School District providing a variety of conflict resolution services.

The Nutrition Outreach and Education Program Assistant assists the NOEP Coordinator in reaching the objective of increasing participation in the Supplemental Nutrition Assistance Program (SNAP) among Steuben County residents.  Responsibilities include providing SNAP outreach & assistance to eligible individuals, increasing awareness about and improving perception of SNAP within the greater community, and developing partnerships with local social service organizations interested in expanding SNAP participation.

The Clinical Supervisor is responsible to the Residential Services Director for the daily operation of the community residence and SLP. Serves as the designated health care coordinator of the programs. The Clinical Supervisor is the immediate supervisor of all staff within the community residence as well as the SLP Counselor.

Pro Action is seeking an outgoing and responsible individual with an excellent driving record to provide the timely delivery of meals to Congregate Meal Sites and Home Delivered Meal customers in Steuben County, from our central kitchen in Bath.

Pro Action is seeking a friendly, outgoing individual to travel to Senior Nutrition meal sites in Steuben and Yates Counties to provide direct customer service and ensure that our customers receive a high quality meal service on a daily basis.
