Register by April 24th: Self-Management for Active Health Summit

You may be aware of recent efforts to build capacity for the delivery of the Chronic Disease Self-Management Program (CDSMP) throughout NYS.  More than 5,000 New York State residents from Buffalo to NYC have participated in this highly acclaimed evidence-based program during the past 18 months and there are currently trainers for the CDSMP in 50% of NYS Counties.  We are hoping you would like to learn more about these efforts, help us to plan for sustainability of the CDSMP and hear both the State and National perspectives on this program and its potential.

To that end, you are cordially invited to attend Self-Management for Active Health, a state-wide summit co-sponsored by the Albany Guardian Society, the NYS Evidence-Based Health Programs Quality & Technical Assistance Center at the University at Albany, NYS Office for the Aging, and NYS Department of Health.  This summit is dedicated to highlighting the impact of the collaborative efforts around the dissemination of the Chronic Disease Self-Management Program (CDSMP).  As we move forward, we are looking for your valued input into how this and other high quality evidence-based programs will be sustained throughout the state.

Day One:  Monday, May 7, 2012, 1 – 5 pm

Program Fidelity Update for Master Trainers in NYS

For NYS Master Trainers and current Program Coordinators only.

Refresher day including program updates, focused discussion, Q&A, and recognition awards!

Day Two:  Tuesday, May 8, 2012, 8 am – 4:30 pm

Self-Management Programs in New York State

For interested stakeholders including health care providers and insurers, aging and health leaders and policy makers, NYS Master Trainers and current Program Coordinators and representatives from the New York State Department of Health and New York State Office for the Aging.  Day two will:

·   Describe the impact of the recently completed American Reinvestment and Recovery Act - U.S. Administration on Aging project to build a sustainable infrastructure for delivery of high quality evidence-based disease health promotion self-management programs in New York State and the value of self-management programs for individuals, communities and systems.

·   Report on the built capacity and outcomes data for the project.

·   Describe sustainability efforts including a discussion of potential reimbursement mechanisms through public and/or private funding and the fit with other health-related and aging-in-place initiatives and systems integration efforts.

·   Seek stakeholder input for a strategic plan to support the built infrastructure as a vehicle for new, different and cross-sector programmatic efforts.
