JOB TITLE: Care Coordinator, Full-Time
SCHEDULE: 8:30 a.m. - 4:30 p.m., Monday-Friday
GRADE: CSEA Schedule B or B-1, Grade 15
             $26.98-$38.21/hour

 

The work involves responsibility for planning, coordinating and supervising the C.A.R.E. (Care Access and Resource Exchange) Unit of the Chemung County Department of Aging and Long Term Care.  The aim of this program is to provide the community in Chemung County with a wide range of long term health care options.  Further, it is to assure that long term health care clients receive appropriate care and services, enabling them to remain in their homes or communities, instead of relying on costly and restrictive institutional care. Work is performed under general supervision by the Director of the Department of Aging and Long Term Care.  The incumbent is responsible for coordinating the functions of all social work, nursing, technical and related staff assigned to the C.A.R.E. Unit, including their training and development. 


OPEN COMPETITIVE QUALIFICATIONS: 
A. Graduation  from  a regionally accredited or New York  State  registered college  or university with a Master's Degree in Human Services,  Public Administration,  a health related field, or a closely related field  and two years  of experience involving the assessment of  client  needs or planning of client services in a health related or human service agency, one year of which must have been in a supervisory capacity; or

B. Graduation  from  a regionally accredited or New York  State  registered college or university with a Bachelor's Degree in Human Services, Public Administration,  a health related field, or a closely related field  and four years of experience as described in (A) above, one year of  which must have been in a supervisory capacity; or

An  equivalent combination of training and experience as defined by  the limits of (A) and (B) above.
