Please sign the SRHN Partnership Agreement and Contact Form

It is time once again for the Network to participate in the competitive RFP process for continued Network funding.  An important part of this process is to show the support of our community partners.  Attached you will find the 2013-2017 Partnership Agreement.  In addition to the Partnership Agreement, you will find a Partnership Contact Form to update your information.  Please complete and return to the Steuben Rural Health Network no later than July 19th.  Forms can be returned via email to jordann@ihsnet.org or by fax at 607-776-9482, Attn. Nicole. 

We ask that each program/service under an organizational umbrella/fiscal agent complete the partnership agreement.  (i.e. Catholic Charities + Steuben Council on Addictions + Kinship, etc would all fill out a partnership agreement).
