The Steuben Rural Health Network held a Peer Leader training for the Chronic Disease Self Management Program through the Stanford University Patient Education Research Center in the month of April.
Eight Peer Leaders representing from a variety of Steuben County nonprofit organizations as well as community volunteers participated in the forty hour certification program where they learned how to empower individuals living with a chronic condition to take charge of their health.
