Pathways Inc. continues to offer Care Management services to individuals with mental health needs during the COVID-19 outbreak.The following services are now offered through the telehealth program.
– Children’s (Health Home) Care Management (https://www.pathwaysforyou.org/children-and-family-services/childrens-care-management): focused on mental health, but can also work with individuals 0-20 with Medicaid and 2 chronic conditions that are physical health needs. Care Managers provide increased support during COVID-19 to assist members to have an outlet / problem-solve issues, in hopes of avoiding the need for contact with emergency rooms / crisis supports that may increase likelihood of COVID-19 exposure. Care Managers also assess and refer youth for Children’s Waiver (Home and Community-Based Services https://www.pathwaysforyou.org/hcbswaiver), which are intensive services to support youth that are at risk of disruption of placement or have intensive needs in order to maintain them in the home. CCM is available in 13 counties in our region.
– Adult (Health Home) Care Management (https://www.pathwaysforyou.org/children-and-family-services/adult-care-management): Care Managers support adults 18+ with Medicaid and Serious Mental Illness or 2 chronic conditions to provide support, prevention, and education to decrease need for services that are higher level (such as emergency rooms, residential treatment, etc). ACM is available in Steuben, Schuyler, and Yates counties. Similar to above, Care Managers in this program can assess and refer Steuben County members for Adult Behavioral Health Home and Community-Based Services (https://www.pathwaysforyou.org/hcbs), who provide individual and more intensive support to members to increase skills and independence.
– Recovery Coordination (https://www.pathwaysforyou.org/children-and-family-services/recovery-coordination): Recovery Coordinator can assess individuals 21 and up who are HARP-eligible for Adult Behavioral Health HCBS. The Recovery Coordinator is not an ongoing support, but rather one-time or periodic assessment to facilitate access to Adult Behavioral Health HCBS.
All services noted above are being provided and facilitated through telehealth (phone or secure video conference). We are adjusting our operations to best support our community with the ever-changing needs and situation we are currently in.
